About Us

The OU Sooner Health Access Network (HAN) started as part of Oklahoma's 1115 Medicaid Waiver designed to support the Patient-Centered Medical Home model serving SoonerCare Choice Members.

The OU Sooner HAN has spent over a decade building a complex care management system to support primary care practices and their members across the state of Oklahoma.

In June 2023, the OU Sooner HAN received National Committee for Quality Assurance (NCQA) accreditation for our Complex Case Management Program. Accreditation was awarded for 3 years and was the first of its kind in Oklahoma.

In April 2024 the OU Sooner HAN expanded to include serving SoonerSelect members and providers enrolled with Aetna Better Health of Oklahoma.

Our Care Managers

Yesenia, RN

It gives me the opportunity to assist members with the little barriers and the life changing barriers they face in the health system and in life in general. The satisfaction of being able to help my members is remarkable every single day.

My favorite part of this job is the connection I get with the members. They come to you with the good and the bad and they know you are there to help them.

Jennifer J., BSN, RN

I have a passion for helping people to achieve their best possible level of health and wellbeing so that they may live their best lives. Being a care manager isn’t just about providing physical assistance, it’s also about nurturing the soul and healing the heart.

I enjoy making in a difference in the lives of my members. Everyone has their own story, and it is rewarding to make an impact. I also enjoy when members tell me I entered their life "at the right time". They didn't ASK for help; I just showed up.

I enjoy the team work attitude with all my colleagues. If I have a question or concern about a Member, I can easily contact any of my team and find the answers. We are a very close family and we all have similar goals which is very helpful.

Care Manager Team Leader

I appreciate the opportunity to have meaningful impact by advocating for members on their behalf. As a patient, it is easy to believe that your voice doesn't matter. As a care manager, I am able to champion my member's voice, encourage its value, and help each member work towards the goals that matter to them.

Jessica, BSN, RN

I like that I can meet my members where they are and work on the goals that are most important to them.

"Da valor a lo pequeño" because as someone who cares, supports, and advocates for others, just as I did for my dad is Care Management. Helping him navigate a world that wasn’t always familiar to him shaped my passion for being a voice for those that do not have one. That is Care Management. In this profession, I learned to appreciate the little things, and to find meaning in small moments of kindness. I love what I do, and knowing it made my dad proud gives it even greater value.
