Care Management

Our Care Management Team has both Registered Nurses and Licensed Clinical Social Workers to assist you.

Care Managers partner with you and your doctor to help you get the most out of your healthcare. 

Your care manager can:

• Answer your questions about your health conditions
• Help you get the healthcare services you need
• Work with you to create a care plan based on YOUR goals
• Help you coordinate your care when you see a specialist or go to the hospital
• Call you to see how you are doing
• Help you find community services like transportation, lower cost medicines, and infant care seats
