Fundamentals of Care Management
The Fundamentals of Care Management course is designed to lead forward thinking care management providers in delivering comprehensive care management services to a diverse population of individuals and populations with high-risk complex health conditions.
About the Course
The next Fundamentals of Care Management Course begins Wednesday May 7, 2025.
This course is FREE for all primary care practices participating with the OU Sooner Health Access Network.

OU Sooner HAN Learning Series
A Monthly Learning Series designed for care managers, providers, and patient care staff. These sessions feature a variety of subject matter experts who present on topics relevant to supporting members living with complex health and social needs.
Each series is 90 minutes long and takes place online, with occasional in-person opportunities. The series is held on the second Friday of each month, starting at 11:30 AM. Social Worker CEUs are available.

DEEPTM
The OU Sooner HAN offers Diabetes Empowerment Education Program also known as DEEPTM to our participating practices and eligible members. DEEPTM is a multi-cultural, bilingual, diabetes self-management education curriculum that has been shown to be successful in helping participants take control of their diabetes and reduce their risk of complications. Since it started in 1996 at the University of Illinois Chicago (UIC) Midwest Latino Health Research, Training and Policy Center, DEEPTM has been offered in all 50 states. The DEEPTM curriculum consists of eight (8) modules that contribute to empowering participants to learn how to successfully live with diabetes. Modules are offered in English and in Spanish.
