What is Care Management?

"A collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet the comprehensive medical, behavioral health and social determinants of health needs of individuals and their families, while promoting quality and cost effective outcomes. The goal of case management is to help people regain optimum health or improved capability, cost effectively and in the right setting."

The OU Sooner HAN Complex Care Management is a person-centered, comprehensive and evidence-based care management program. OU Sooner HAN Care Managers (RN's and LCSW's) work with complex members to engage them in informed decision-making and shared care planning. This includes comprehensive health and social determinants assessments, transition services, medication reconciliation, education, coaching and self-management, and access to community resources. Services are delivered through in-home visits, telephonic, hospital visits, clinic visits, and telehealth tools.

Fundamentals of Care Management

The Fundamentals of Care Management course is designed to lead forward thinking care management providers in delivering comprehensive care management services to a diverse population of individuals and populations with high-risk complex health conditions.

OU Sooner HAN Learning Series

A Monthly Learning Series designed for care managers, providers, and patient care staff. These sessions feature a variety of subject matter experts who present on topics relevant to supporting members living with complex health and social needs.

Each series is 90 minutes long and takes place online, with occasional in-person opportunities. The series is held on the second Friday of each month, starting at 11:30 AM. Social Worker CEUs are availble

DEEP Training

The OU Sooner HAN offers Diabetes Empowerment Education Program also known as DEEPTM to our participating practices and eligible members. DEEPTM  is a multi-cultural, bilingual, diabetes self-management education curriculum that has been shown to be successful in helping participants take control of their diabetes and reduce their risk of complications. Since it started in 1996 at the University of Illinois Chicago (UIC) Midwest Latino Health Research, Training and Policy Center, DEEPTM has been offered in all 50 states.   The DEEPTM curriculum consists of eight (8) modules that contribute to empowering participants to learn how to successfully live with diabetes. Modules are offered in English and in Spanish.

These trainings are designed to address practice-relevant topics, such as diabetes management, asthma interventions, or enhancing cultural consciousness in patient care. By working closely with your team, we ensure that each training is aligned with the unique challenges and goals of your practice, empowering your staff with the skills and knowledge needed to provide optimal care for your patients.

Our goal is to enhance quality knowledge, streamline operations, improve member care and outcomes, boost employee morale, and increase patient satisfaction.
