Connect with Care Management

Our Care Management Team has both Registered Nurses and Licensed Clinical Social Workers to assist you. Care Managers partner with you and your doctor to help you get the most out of your healthcare. 

Your care manager can

• Answer your questions about your health conditions
• Help you get the healthcare services you need
• Work with you to create a care plan based on YOUR goals
• Help you coordinate your care when you see a specialist or go to the hospital
• Regular check-in calls to see how you are doing
• Help connect you to community services that can assist with things like housing, food, transportation, lower cost medicines, and more.

How to Request a Care Manager

In addition to speaking with your Primary Care Provider, you can request a care manager by calling (918) 660-3067 or email SoonerHAN@ouhsc.edu.
